Overview

Goals LiveWell Community Wellness Grants aim to support the health of Douglas County residents by creating conditions that lead to healthy eating and physically active lifestyles. Projects should help the community make progress toward achieving goals set forth in the 2018 – 2023 Douglas County Community Health Plan specifically related to Food Security and Healthy Built Environment goals. Grants will help Douglas County organizations: adopt policy, systems and environmental changes that support healthy eating and physical activity in everyday environments such as childcare centers, schools, after-school programs, workplaces, community centers, neighborhoods and places of worship, oroffer programs that help residents make lasting improvements to their diets and activity patterns that reduce their risk for lifestyle-related chronic diseases such as hypertension, heart disease, diabetes and cancer. Grants will also foster collaboration and a sense of shared purpose among organizations committed to working together to improve community health. Resources Most of the amount DCCF has available for this grant competition comes from funds focused on children’s health. We expect to award: Up to $80,000 for projects that primarily target children ages 0-18 and their familiesUp to $20,000 for projects that primarily target adults or the general publicGrants are expected to range from $1,000-$15,000.

Proposal must come from a 501(c)(3) nonprofit organization or government entity.Proposal must be for work that primarily benefits residents of Douglas County, Kansas.Applicants must conduct business without discrimination on the basis of race, religion, gender, sexual orientation, age, marital status, disability or national origin.Projects must help Douglas County communities make progress toward achieving the goals and anticipated outcomes set forth in the Food Security and Healthy Built Environment sections of the 2018 – 2023 Douglas County Community Health Plan.
