Implementation of Outpatient Clinical Pharmacy Services - Award for a Pharmacist and Pharmacy Technician
This program is intended to facilitate development and implementation of outpatient clinical pharmacy services at care centers and affiliate programs accredited by the Cystic Fibrosis Foundation.
The focus of this Request for Applications (RFA) is to provide seed funding and travel support to secure a pharmacist and a pharmacy technician who will be responsible for the development and implementation of outpatient clinical pharmacy services at care centers and affiliate programs accredited by the Cystic Fibrosis Foundation.
For those programs that have an established outpatient clinical pharmacy service, this award is not intended to replace the support already in place. However, the Foundation will consider an application from such programs to secure additional funding for a pharmacist and/or a pharmacy technician to optimize clinical pharmacy services.
Funding
The allowable award amount is based on program size. Applicants working at a care center with >100 patients in the CF patient registry may request up to $100,000 over two (2) years to support the salary and benefits for a Pharmacist AND a Pharmacy Technician, plus twelve (12) percent indirect costs. Applicants working at a center with 50-100 patients in the registry may request funding of up to $75,000 total spread out over a period of two (2) years, plus an additional twelve (12) percent indirect costs. The mission of the Cystic Fibrosis Foundation is to cure cystic fibrosis and to provide all people with CF the
opportunity to lead long, fulfilling lives by funding research and drug development, partnering with the CF
community, and advancing high-quality, specialized care.
To achieve this mission, various types of grants and awards are offered to support care centers, career
development as well as meritorious research in CF.
II. Program and Award Overview
Current CF Foundation Care Center Accreditation guidelines recommend having a pharmacist as a member
of the CF Center multidisciplinary team. In 2016, the CF Foundation Strategic Investment Committee
recommended developing outpatient pharmacy services at CFF-Accredited Care Centers. To date, one-
hundred seventeen programs have received this award and one-hundred fifteen have been able to sustain
the positions following completion of the award period.
This Award is designed to facilitate development and implementation of outpatient clinical pharmacy
services at CFF-Accredited Care Centers and Affiliate programs throughout the U.S. that currently lack a
pharmacist and pharmacy technician as integral members of the multidisciplinary care team. The focus of
this Request for Applications (RFA) is to provide seed funding and travel support to secure a pharmacist
and pharmacy technician who will be responsible for the development and implementation of outpatient
clinical pharmacy services which includes but is not limited to:
• Ensuring safe, appropriate, cost-effective therapy;
• Assessing drug dosing, drug-drug interactions, drug allergies, adverse drug reactions;
• Advising on CF and non-CF medication therapy;
• Providing drug information and/or counseling to patients and/or caregivers;
• Serving as liaison between CF clinic, hospital or institution, community practitioners, and payers;
• Assess with patient/family the sustainability of the medication regimen and develop strategies to
improve medication management; and
• Evaluating and assisting in ensuring access to medications.
For those programs that have an established outpatient clinical pharmacy service, this award is not
intended to replace the support already in place.

data on the impact of the clinical pharmacy services provided and ensure that pharmacy assessments
are captured in the CF Foundation Patient Registry for each patient counseled.
• Sites receiving awards will submit an annual progress report to document progress and fulfillment of
award requirements including documentation of the clinical pharmacy services provided and
compilation of data on subsequent assessments, interventions, and outcomes.
Year 1 Progress Report should provide specific details on the contributions by awardee to

implement outpatient clinical pharmacy services and follow-up.
Year 2 Final Report

• Funding for Year 2 is contingent upon progress documented in the Year 1 Progress Report.

VII. Review and Reward
The pharmacist and pharmacy technician do not have to be identified at the time of the application.
Applications will be reviewed by a peer review committee. If your program chooses not to request the full
funding amount for both a pharmacist and a pharmacy technician, you will need to provide justification for
your decision. This justification will be considered by the peer review committee.
Applications will be scored based on:
a) Feasibility of plan to implement outpatient clinical pharmacy services.
b) Sustainability of the pharmacist and pharmacy technician positions following this 2-year award.
c) Integration of both a pharmacist and a pharmacy technician is preferred. If a pharmacy technician is
already in place, that should be stated.

Chief causes for assigning low priority scores to applications during review include the following:
• Insufficient information or documentation
• Lack of focus on outpatient setting
• Lack of clear plan to sustain pharmacy services post-award period
• Failure to meet all of the criteria described in this policies and guidelines statement
Applications will be reviewed by a CF Foundation ad hoc review committee. Funding decisions are based
upon the availability of funds, responsiveness to the RFA and eligibility.

Complete the required contact fields by searching by name for existing contacts at your institution for each

BUDGET
Select the “Edit Budget” button under Application Budget, to enter and begin completion of the
application’s budget detail for each year of funding being requested. All Awards are for a maximum of two
years. The total maximum budget for both years cannot exceed $125,440 (which includes 12% indirect
costs) total for a program of >100 patients including the salaries & benefits for both the pharmacist and
pharmacy technician, NACFC travel expenses, and registration fees.
Salaries & Benefits - List the names, positions, and percent effort for the applicant. Be sure to complete
all fields on the Budget Detail in full on the template provided. In accordance with National Institutes of
Health (NIH) policy, salary requests may not use an institutional base salary in excess of the current
federal salary cap ($221,900 in 2024). Fringe benefits may be requested if they are treated consistently
by the applicant institution as a direct cost to all funding agencies and foundations.

The applicant may choose to allocate the salary support variably over the two-year period. For example,
the year 1 salary request could be $30,000 for planning with a hire or adjustment in FTE later in year 1,
and then a higher amount ($70,000) requested for year 2. Alternatively, a center may already have an
individual identified for the pharmacist and/or pharmacy technician role and choose to budget $50,000
equally over 2 years or $100,000.

Travel – Travel funding can only be used for NACFC. Travel expenses may not exceed $3,000 per person

The CF Foundation defines “Key Project Personnel” as any individual with an advanced degree who will
play an instrumental role in the research project. An NIH Biographical Sketch form should be completed
for each key project personnel and uploaded as PDF. The maximum length for each biosketch is five (5)
pages.

team and the specific roles/tasks that might be offloaded, provided, and/or reallocated by
adding a pharmacist and pharmacy technician?
(4) If the pharmacist and pharmacy technician is unfamiliar with CF, what training plan will you
put in place to provide the basics needed for them to function effectively as a multidisciplinary
team member in the clinic?
3) Sustainability
a) Describe your clinic and/or health system plan to obtain the necessary funding to sustain the
pharmacist and pharmacy technician positions following this 2-year award (e.g., health system
pharmacy or specialty pharmacy with or without 340b contract pricing) (Maximum-two pages)

OTHER SUPPORT (NIH FORMAT)
Complete and upload the Other Support form for all key project personnel, beginning with the
Applicant/Principal Investigator. There is no page limitation. Information on other support assists CFF in
the identification and resolution of potential sources of overlap. Scientific and budgetary overlap should
be minimized. Commitment of an individual’s effort greater than 100 percent, is not permitted.
LETTERS OF SUPPORT
A minimum of three (3) letters of support are required for submission. Letters of support should be
submitted per the following:
• A letter of support from the Institution’s Pharmacy Services Director or equivalent
The letter must confirm that the pharmacist and pharmacy technician will have adequate

protected time to participate fully in the program.
• Other letters of reference (if pharmacist is identified)
Up to two letters of reference from individuals who are familiar with the pharmacist’s prior

work record, interests, and abilities related to CF.
Letters uploaded to https://awards.cff.org/ should not be password protected or otherwise encrypted.
Such encryption will cause errors in assembling a single-print PDF of the application. The applicant should
inform the individuals writing letters to not include password protection on their documents.

recommended developing outpatient pharmacy services at CFF-Accredited Care Centers. To date, one-
hundred seventeen programs have received this award and one-hundred fifteen have been able to sustain
the positions following completion of the award period.
This Award is designed to facilitate development and implementation of outpatient clinical pharmacy
services at CFF-Accredited Care Centers and Affiliate programs throughout the U.S. that currently lack a
pharmacist and pharmacy technician as integral members of the multidisciplinary care team. The focus of
this Request for Applications (RFA) is to provide seed funding and travel support to secure a pharmacist
and pharmacy technician who will be responsible for the development and implementation of outpatient
clinical pharmacy services which includes but is not limited to:
• Ensuring safe, appropriate, cost-effective therapy;
• Assessing drug dosing, drug-drug interactions, drug allergies, adverse drug reactions;
• Advising on CF and non-CF medication therapy;
• Providing drug information and/or counseling to patients and/or caregivers;
• Serving as liaison between CF clinic, hospital or institution, community practitioners, and payers;
• Assess with patient/family the sustainability of the medication regimen and develop strategies to
improve medication management; and
• Evaluating and assisting in ensuring access to medications.
For those programs that have an established outpatient clinical pharmacy service, this award is not
intended to replace the support already in place.
III. Funding Amounts
Salary Support for Pharmacist and Pharmacist Technician over the 2-year period.
The allowable award amount is based on program size.
• Applicants working at a care center with >100 patients in the CF patient registry may request up to
$100,000* total over two (2) years to support the salary and benefits for a Pharmacist AND a Pharmacy
Technician, plus twelve (12) percent indirect costs.
• Applicants working at a center with 50-100 patients in the registry may request funding of up to
$75,000* total over two (2) years to support the salary and benefits for a Pharmacist AND a Pharmacy
Technician, plus twelve (12) percent indirect costs.
• Applicants working at a center with <50 patients in the registry may request funding of up to $50,000*
total over two (2) years to support the salary and benefits for a Pharmacist AND a Pharmacy
Technician, plus twelve (12) percent indirect costs.
*Does not have to be evenly divided between years.

IV. Eligibility
• Applicants must be from a non-profit or academic institution; for-profit entities are not eligible to
apply. For-profit entities should visit Industry Funding Opportunities for more information.
• Applicants must be U.S. Citizens or U.S. permanent residents (must have obtained permanent
residency prior to the time of application)
• The principal investigator must be a CF center director, program director, or an associate director.
• Award for a pharmacist and a pharmacy technician must go towards salary support and travel for a
pharmacist and pharmacy technician who will focus on providing outpatient clinical pharmacy services
to the CFF-accredited program.
• Supported personnel will commit to providing outpatient clinical pharmacy services. They will collect
data on the impact of the clinical pharmacy services provided and ensure that pharmacy assessments
are captured in the CF Foundation Patient Registry for each patient counseled.
• Sites receiving awards will submit an annual progress report to document progress and fulfillment of
award requirements including documentation of the clinical pharmacy services provided and
compilation of data on subsequent assessments, interventions, and outcomes.
Year 1 Progress Report should provide specific details on the contributions by awardee to
o
implement outpatient clinical pharmacy services and follow-up.
Year 2 Final Report
o
• Funding for Year 2 is contingent upon progress documented in the Year 1 Progress Report.

VII. Review and Reward
The pharmacist and pharmacy technician do not have to be identified at the time of the application.
Applications will be reviewed by a peer review committee. If your program chooses not to request the full
funding amount for both a pharmacist and a pharmacy technician, you will need to provide justification for
your decision. This justification will be considered by the peer review committee.
Applications will be scored based on:
a) Feasibility of plan to implement outpatient clinical pharmacy services.
b) Sustainability of the pharmacist and pharmacy technician positions following this 2-year award.
c) Integration of both a pharmacist and a pharmacy technician is preferred. If a pharmacy technician is
already in place, that should be stated.

Chief causes for assigning low priority scores to applications during review include the following:
• Insufficient information or documentation
• Lack of focus on outpatient setting
• Lack of clear plan to sustain pharmacy services post-award period
• Failure to meet all of the criteria described in this policies and guidelines statement
Applications will be reviewed by a CF Foundation ad hoc review committee. Funding decisions are based
upon the availability of funds, responsiveness to the RFA and eligibility.

preloaded as the Lead Institution. Domestic applicants must verify their institution by entering the
Employer Identification Number (EIN) or Tax Identification Number (TIN) to search the system for the
correct institution. If the EIN/TIN is not located, you may add the legal institution. Please also confirm if
the project site is the same as the legal institution.
Verification of Applicant Institution’s Tax Status (upload as PDF documents):
The CFF GCMA Office must have a copy of the applicant institution’s current W-9 and 501©3 letter, or
other documentation verifying its Federal tax status and will not issue Award Letters to Awardees if these
documents are not received and on file.
• Applicants from for-profit organizations must submit a copy of the applicant institution’s W-9 and IRS
documentation verifying the organization’s Federal tax status. Awards are not issued prior to having
these documents on file with the CFF GCMA Office.

CONTACTS
Please note: The INSTITUTION tab must be completed prior to adding internal contacts to ensure that
the contacts are properly associated with the applicant institution.
Complete the required contact fields by searching by name for existing contacts at your institution for each
role. If the desired institutional contact is not available in the system, you may select “Add Internal
Contact” to create a basic contact profile in order to add the individual to your application.
BUDGET
Select the “Edit Budget” button under Application Budget, to enter and begin completion of the
application’s budget detail for each year of funding being requested. All Awards are for a maximum of two
years. The total maximum budget for both years cannot exceed $125,440 (which includes 12% indirect
costs) total for a program of >100 patients including the salaries & benefits for both the pharmacist and
pharmacy technician, NACFC travel expenses, and registration fees.
Salaries & Benefits - List the names, positions, and percent effort for the applicant. Be sure to complete
all fields on the Budget Detail in full on the template provided. In accordance with National Institutes of
Health (NIH) policy, salary requests may not use an institutional base salary in excess of the current
federal salary cap ($221,900 in 2024). Fringe benefits may be requested if they are treated consistently
by the applicant institution as a direct cost to all funding agencies and foundations.

The applicant may choose to allocate the salary support variably over the two-year period. For example,
the year 1 salary request could be $30,000 for planning with a hire or adjustment in FTE later in year 1,
and then a higher amount ($70,000) requested for year 2. Alternatively, a center may already have an
individual identified for the pharmacist and/or pharmacy technician role and choose to budget $50,000
equally over 2 years or $100,000.

Travel – Travel funding can only be used for NACFC. Travel expenses may not exceed $3,000 per person
per year including NACFC registration fees. Registration fees associated with NACFC must be listed under

the “Other Expenses” category. Awardees are encouraged to attend the North American CF Conference
each year.
Other Expenses – Other Expenses are limited to registration fees for NACFC conference attendance.

BUDGET JUSTIFICATION

Justify all items, providing detail and reasoning for budgeting each item. Make sure amounts and figures
listed in the narrative are consistent with those listed in the Budget Detail. *Budget Justification

BIOGRAPHICAL SKETCH(ES) OF KEY PERSONNEL (For pharmacist, if identified)
The CF Foundation defines “Key Project Personnel” as any individual with an advanced degree who will
play an instrumental role in the research project. An NIH Biographical Sketch form should be completed
for each key project personnel and uploaded as PDF. The maximum length for each biosketch is five (5)
pages.

team and the specific roles/tasks that might be offloaded, provided, and/or reallocated by
adding a pharmacist and pharmacy technician?
(4) If the pharmacist and pharmacy technician is unfamiliar with CF, what training plan will you
put in place to provide the basics needed for them to function effectively as a multidisciplinary
team member in the clinic?
3) Sustainability
a) Describe your clinic and/or health system plan to obtain the necessary funding to sustain the
pharmacist and pharmacy technician positions following this 2-year award (e.g., health system
pharmacy or specialty pharmacy with or without 340b contract pricing) (Maximum-two pages)

OTHER SUPPORT (NIH FORMAT)
Complete and upload the Other Support form for all key project personnel, beginning with the
Applicant/Principal Investigator. There is no page limitation. Information on other support assists CFF in
the identification and resolution of potential sources of overlap. Scientific and budgetary overlap should
be minimized. Commitment of an individual’s effort greater than 100 percent, is not permitted.
LETTERS OF SUPPORT
A minimum of three (3) letters of support are required for submission. Letters of support should be
submitted per the following:
• A letter of support from the Institution’s Pharmacy Services Director or equivalent
The letter must confirm that the pharmacist and pharmacy technician will have adequate

protected time to participate fully in the program.
• Other letters of reference (if pharmacist is identified)
Up to two letters of reference from individuals who are familiar with the pharmacist’s prior

work record, interests, and abilities related to CF.
Letters uploaded to https://awards.cff.org/ should not be password protected or otherwise encrypted.
Such encryption will cause errors in assembling a single-print PDF of the application. The applicant should
inform the individuals writing letters to not include password protection on their documents.
