Overview
Community Health Grant Grants are awarded to free and charitable clinics, school-based health centers and community health centers nationwide to increase access to care and improve health outcomes for people of all ages. The CVS Health Foundation has made a multi-million commitment to expanding access to quality health care nationwide through partnerships with the National Association of Free & Charitable Clinics (NAFC) and the National Association of Community Health Centers (NACHC). In 2016, grants have been awarded to free and charitable clinics and community health centers nationwide to increase access, improve health outcomes and lower overall health costs across the country in innovative and strategic ways.Eligibility
