Crisis Intervention Fund

Purpose: To provide financial assistance to persons with kidney disease who are on dialysis or who have had a kidney transplant and reside in the state of Louisiana.

Geographic Scope: Louisiana only

Program Type: One-time emergency grant

Eligibility

• Persons with kidney disease on dialysis OR
• Persons with a kidney transplant
• Reside in Louisiana
• Experiencing an unexpected monthly expense due to a crisis

What the Fund Covers

• One-time grant designed to resolve a specific crisis
• Not intended for ongoing financial assistance

Application Requirements

• Applications available to social workers only
• Social workers apply on behalf of the patient
• Must demonstrate that all other known community resources and family support have been explored
• Program serves as a last resort option

How to Apply

1. For Social Workers Only: Social workers submit applications on behalf of kidney patients
2. Request Access: Contact Tracey Eldridge at tracey@kidneyla.org to receive access to the password-protected application
3. Complete Application: Fill out the Crisis Intervention Fund form
4. Documentation Required:
   • Verification of kidney disease status (dialysis or transplant)
   • Description of the crisis/unexpected expense
   • Documentation that all other community resources and family support have been explored
5. Submit: Submit completed form to Tracey Eldridge
