CORE Grant Program
Full Name: Community Outreach, Recruitment, and Engagement (CORE) Fund
Overview: Friends of CMH Foundation accepts grant requests to help team members enhance the patient experience. Grant requests are accepted from all departments to further the mission, vision and values of Community Memorial Hospital – UnityPoint Health and are expected to align and support the strategic plan.
Award Amount: A $5,000 grant is awarded during each review period.
Review Schedule: Grant requests are reviewed semi-annually in May and November, but applications can be submitted at any time.
Eligibility: Open to all departments of Community Memorial Hospital.
Focus: Projects that enhance the patient experience and align with the hospital's mission, vision, values, and strategic plan.
Past Recipients: Fall 2021 - Friends of CMH delivered equipment to departments at Community Memorial Hospital as a result of the CORE grant program. The program generated funds from seven different grants and allowed for the purchase of an Ergotron Sit-Stand Mobile Desk and Hi-Lo Exam table and chair.
