NOTE: All first-time applicants are required to meet with the Community Partnership Fund Team before submitting an application. First-time applicants should meet with the Community Partnership Fund team at least one week before the application cycle closes. About Us Adventist HealthCare, based in Gaithersburg, Md., is a faith-based, not-for-profit organization of dedicated professionals who work together each day to improve the health and well-being of people and communities through a ministry of physical, mental and spiritual healing. Community Partnership Fund Adventist HealthCare is committed to addressing the needs of the communities we serve. Through our Community Partnership Fund (CPF), we provide funding to organizations whose work promotes health equity and wellness in our community. Funding Priorities Our Community Partnership Fund Funding Priorities will focus on significant needs identified through our Community Health Needs Assessment. To be eligible for funding, requests must align with at least one of the three areas below: Access to Care: Expand access to comprehensive healthcare and wellness servicesPrograms addressing access to primary care and specialty care, including mental and behavioral healthPrograms addressing low-income, uninsured, and underinsured populationsHealthy Behaviors: Encourage individuals to engage in healthy behaviorsPrograms enabling access to and completion of preventive and early-detection health screeningsPrograms addressing food insecurity, obesity, and physical inactivityPrograms addressing chronic disease prevention and management, specifically for diabetes and heart diseaseEducation, Income, & Workforce: Build equitable communitiesPrograms addressing educational inequities (e.g., kindergarten readiness, high school completion or college preparation)Programs supporting attainment of employment with a livable wage (e.g., pipeline programs, career coaching and mentorship)Programs that address housing stability, quality, and safety (e.g., creating safe living conditions and assisting individuals exiting homelessness) Program Grants Amounts more than $5,000.

Ineligibility

Organizations that do not comply with submitting progress and/or final reports by the submission deadlines will be ineligible to apply for any funding opportunities in the subsequent year.
