Community Partnership Fund: Program Grants

The Community Partnership Fund by Adventist HealthCare offers program grants over $5,000 to 501(c)(3) nonprofit organizations addressing health disparities in Montgomery and Prince George’s Counties. Applicants must demonstrate alignment with key health priorities, including access to care, promoting healthy behaviors, and educational equity. This initiative aims to enhance community wellness and equity through measurable impacts in underserved populations. First-time applicants are required to consult with the Community Partnership Fund Team before applying.

All first-time applicants are required to meet with the Community Partnership Fund Team before submitting an application. First-time applicants should meet with the Community Partnership Fund team at least one week before the application cycle closes.
Adventist HealthCare is committed to addressing the needs of the communities we serve. Through our Community Partnership Fund (CPF), we provide funding to organizations whose work promotes health equity and wellness in our community.
Funding Priorities
Our Community Partnership Fund Funding Priorities will focus on significant needs identified through our Community Health Needs Assessment.
To be eligible for funding, requests must align with at least one of the three areas below:
Access to Care: Expand access to comprehensive healthcare and wellness servicesPrograms addressing access to primary care and specialty care, including mental and behavioral healthPrograms addressing low-income, uninsured, and underinsured populationsHealthy Behaviors: Encourage individuals to engage in healthy behaviorsPrograms enabling access to and completion of preventive and early-detection health screeningsPrograms addressing food insecurity, obesity, and physical inactivityPrograms addressing chronic disease prevention and management, specifically for diabetes and heart diseaseEducation, Income, & Workforce: Build equitable communitiesPrograms addressing educational inequities (e.g., kindergarten readiness, high school completion or college preparation)Programs supporting attainment of employment with a livable wage (e.g., pipeline programs, career coaching and mentorship)Programs that address housing stability, quality, and safety (e.g., creating safe living conditions and assisting individuals exiting homelessness)
Amounts more than $5,000.

Eligibility

To be eligible for funding, requests must:Originate from a 501(c)3 not-for-profit organizationFocus on populations in Adventist HealthCare’s services areas of Montgomery and Prince George’s CountiesAddress documented health disparities or inequitiesHave a measurable impact on the community being servedAlign with one or more of the Community Partnership Fund Priority AreasOrganizations are eligible for only one sponsorship or grant per year.For returning grant and sponsorship recipients:Past performance will be considered in reviewing subsequent funding requests including determining successful implementation of programming as outlined in the funding request; achievement of program objectives; and timely submission of thoughtful progress and final reports.

Ineligibility

Organizations that do not comply with submitting progress and/or final reports by the submission deadlines will be ineligible to apply for any funding opportunities in the subsequent year.
