New Jersey Department of Health The scope of work for the public health system is ever-expanding, and the Department is on the frontlines in leading the response to public health challenges. The Department has three primary branches – Public Health Services, Health Systems, and Integrated Health. All work collaboratively toward improving health by strengthening New Jersey’s health system. Our Mission: Protect the public’s health, promote healthy communities, and continue to improve the quality of health care in New Jersey Community Health Worker CHW Hub NOFA Purpose For Which The Grant Program Funds Shall Be Used The CHW Hub will be responsible for ensuring NJ has a strong CHW workforce by working with organizations across the state in continuing the ongoing certification efforts for CHWs and sustainability initiatives for the CHW workforce. In partnership with NJDOH, the CHW Hub will also actively support and advocate for the integration of CHWs into standard healthcare practice. Award Period: From 9/1/25 Through 7/31/26

Group or Entities Which May Apply For The Grant ProgramMunicipal Government; County Gove; Institution of Higher Education; Hospital; Non-profit Organization (501(c)3 Qualifications Needed By Applicant To Be Considered For A Grant: Public agency or private non-profit organization with the capacity and ability to meet required administrative, programmatic and fiscal processes necessary to develop the infrastructure and related sercices for the implementation of the project. In addition to funding availability, continuation of funding for subsequent years will be contingent upon timely accurate submission of reports, an approved annual plan, and satisfactory progress toward completion of the current year's grant objectives. A detailed narrative desciption of how the applicant organization is best fit to create the CHW Hub is also required. This applicant organization must have extensive background working with managed care organizations (MCOs).
