2024-2025 Child Neurology Foundation Care Advancement Grants in Transition of Care and Care Coordination $26,000 Grant for Care Advancement/Quality Improvement in Transition of Care or Care Coordination CNF will be awarding thirteen (13) grants of $26,000 each to institutions and/or clinics for projects improving the patient care experience related to transition of care or care coordination.
