Chapter Restricted Fund Grant Request

Application Process

1. Complete Online Form at https://akl.org/akl-education-foundation/scholarships-grants/chapter-restricted-fund-grant-request/

2. Required Information:
- Chapter name and initiation status
- Contact information (name, email, phone, address)
- Grant amount requested
- Purpose of grant (select from: academics, leadership program attendance, or other)

3. Recipient Details:
- For academic scholarships: List all recipients with full permanent mailing address and amount each should receive
- For leadership program scholarships: List recipients, program name, mailing address, and amount
- For other purposes: Provide detailed description demonstrating educational purpose under IRS regulations, recipient information, and amounts

4. Scholarship Delivery:
- AKL Education Foundation will mail checks directly to recipients
- Checks can be sent to alumni/volunteer representative if presenting at event

5. Agreement:
- Sign digital signature acknowledging Chapter Restricted Fund Agreement
- Confirm understanding that funds are for sole educational purposes requested
- Acknowledge that misuse results in disqualification and fund repayment

6. Submit Form with digital signature and date
