CFNJ: Somerset Hills Community Health Foundation Grant

The Somerset Hills Community Health Foundation Grant, offered by the Community Foundation of New Jersey, aims to fund projects that enhance health and well-being in Morris and Somerset counties. The foundation prioritizes initiatives that promote independent living for underserved populations, address chronic health issues, and support healthcare workforce development. By focusing on community-driven and equity-informed projects, the grant seeks to create lasting positive change for historically underserved communities.

Grantmaking Philosophy
The Community Foundation of New Jersey considers regularly how we as an organization and civic community leader can effect lasting change that leads to greater equity. To that end, our discretionary grantmaking is informed by many of the below criteria and grounded in the associated values.
Criteria
We seek to support initiatives that:

• Pilot a good idea and have a realistic path toward broader uptake
• Have an achievable public policy objective
• Promote a public systems fix that CFNJ and partners have the tools/standing to influence
• Reach a New Jersey community or group of people who are not being served or have historically been underserved.
• Leverage other private or public resources
• Values
• The work we support is:
• Community-driven, with input that comes from the community and take into account the expressed needs and plans of the community.
• Equity-informed, directly addressing racial, economic and/or social inequity in New Jersey.

Diversely-led, drawing from a greater diversity of organizations that people of color, different ages and perspectives lead and/or serve, and that may or may not have proven track records but show promise in future work
Broad-based, serving more than one community.
The Somerset Hills Community Health Foundation (SHCHF) seeks to fund projects and programs that promote healthier individuals and communities in Morris and Somerset counties.
Grants are awarded to organizations whose proposals address one or more of the following Foundation priorities:
Foster independent living through innovative programs for underserved and/or aging population.Support critical community health and wellness needs addressing chronic disease prevention and management, healthy aging and mental health.
Support programs to increase the number of available healthcare workers (i.e., nurse, therapist, home health aide) through scholarships and training (i.e., degrees, certifications, continuing education).

Eligibility

Please note, SHCHF is committed to promoting healthy individuals and communities in Morris and Somerset counties. To that end, we exclusively fund programs that serve Morris and Somerset counties. If your program serves multiple counties, we request that you have the capacity to carve out and specifically track the services you provide in these two counties. If you do not currently have that capacity, we respectfully request that you seek other sources of funding.

Ineligibility

We do not fund affordable housing and capital improvements.It is the intention of the Foundation to avoid directly or indirectly conducting or supporting activities that compete with the services or fundraising of Atlantic Visiting Nurse or the Atlantic Health System.
