How to Donate and Receive Match

1. Online Donation:
- Visit the campaign donation page
- Submit gift online (eligible for immediate match documentation)

2. By Check:
- Make checks payable to: Carmel Richmond Nursing Home
- Mail to: 88 Old Town Rd, Staten Island, NY 10304
- Write in memo section: Diamond Matching Grant

3. Employer Matching:
- Check with HR/benefits team for corporate matching gift program
- Employer match dollars count as additional resources and stack on Foundation's 2:1 match

4. Documentation:
- Gifts must be documented by June 30, 2026 to be eligible for match
- Receipt and thank you letter will be provided
- Email receipt from Catholic Health Care Foundation of the Archdiocese of New York
- Personalized thank you from Carmel Richmond
