- Review criteria include student motivation, personal goal achievement, household management skills, employment goals, and community engagement

1. Household & Self-Reliance Skills: Describe what skills you still need to learn or practice to become more self-reliant in your everyday life.
2. Employment Goals: Explain why being employed is important to you. Name three jobs you would like to have in the future.
3. Roommate Living: Describe what you think you will like most about having roommates and what challenges you might face living with roommates.
4. Program Motivation: Explain in a few sentences why you would like to attend the Transition Academy.
5. Community Engagement: What does "community" mean to you? How do you plan to connect to your community? How do you plan to find outlets for your personal interests and passions? (Example: music–attend live music events; gaming–host and/or attend gaming events; etc.)
