Backus Hospital Commemorative Auxiliary Scholarship
Overview: A one-time award established to support dependents of Backus Hospital colleagues and past/present volunteers who are furthering or pursuing a career in healthcare-related roles.
Geographic Scope: Connecticut, US (Backus Hospital facilities)

Eligibility Requirements
- Applicant Status: Dependent of a Backus colleague/colleague who primarily works at a Backus facility, OR current or former Backus volunteer
- Dependent Definition: Individuals claimed on your taxes as defined by the IRS; child must live in the same home as you in the United States for more than half of the tax year (https://www.irs.gov/credits-deductions/individuals/earned-income-tax-credit/qualifying-child-rules)
- Education Status: Must be a high school graduate, high school graduate by June of the current academic year, and/or enrolled in an accredited post-secondary school
- Field of Study: Must be pursuing studies in a healthcare-related field
- Application Submission: Must submit a completed application
- Documentation: Must provide current transcript (or evidence of GPA); updated resume is optional
Selection Criteria
The Backus Hospital Commemorative Auxiliary Scholarship Committee selects up to five scholarship recipients based on:

- Early/Late Applications: Will not be accepted or reviewed
- Decision Notification: By April 10
- W-9 Submission Deadline: April 30 (required for award disbursement)
- Failure to Comply: Results in application rejection

- Payment Method: Checks made payable to recipient's account at their university/college Bursar's office

How to Apply
Application Process
1. Submission Period: Applications accepted March 9 - March 29 only (no early or late applications accepted)
2. Required Materials:
- Completed application form
- Current transcript or evidence of GPA
- Updated resume (optional)
3. Notification: Committee decision by April 10
4. Post-Award Requirements: Upon notification of award, complete W-9 form for identified college/university
5. Deadline for W-9 and Paperwork: Submit to Backus.Volunteers@hhchealth.org no later than April 30, 2026
6. Consequence: Failure to comply with timeline results in Committee redacting your application
