Eligibility:

• Must have a cancer diagnosis (verified)
• Must be a resident of VA, MD, or D.C.
• Teenagers and young adults

What is Required:

• Essay or video submission
• Short answer responses
• Diagnosis verification documentation
• Proof of residency
• Community Service Activity Log

Application Details:

• Electronic submission only through website
• Paper and mailed applications will not be accepted
• Application must be fully completed before submission
• Cannot save progress and return later
