1. Obtain application form from memo distributed by LDH Deputy Assistant Secretary
2. Complete application as a certified nursing home
3. Submit application to LEADER
4. Prepare supporting documentation including invoices and receipts for technology purchases
5. LEADER reviews supporting documents
6. Upon LDH approval, LEADER processes reimbursement payment
