    Since 1968, Wyoming County United Way has raised funds during annual campaigns to support health and human service programs that meet the needs and improve the lives of Wyoming County residents.

    Required Information

    The application requires submission of the following in 11 sections:

    1. Agency Information: Name, address, website, contact person details
    2. Mission: Agency mission statement
    3. Programs: Description of agency programs and target populations
    4. Funding Requests: Up to 3 programs with names, descriptions, and requested amounts
    5. Service Area: Description of Wyoming County service area and client base
    6. Funding Sources: Details of all agency funding sources
    7. Expenses: Program budget and expense breakdown
    8. Impact Narrative: Brief explanation of how United Way funding will be used
    9. Community Impact (Inputs): Inputs, activities, and outputs for up to 2 outcomes
    10. Community Impact (Outcomes): Outcomes and indicators with data sources
    11. Stories: Success stories (optional for marketing use)
