TPREF Patient Care Program - Pharmacist-Provided Diabetes and Hypertension Care

Funder: Tennessee Pharmacists Research and Education Foundation (TPREF), funded by Centers for Disease Control and Prevention (CDC) and Tennessee Department of Health (TDH)

Grant Award: $750,000 total grant from CDC and TDH to TPREF

    Primary Goals

  • Train and credential 100 pharmacists through the American Association of Diabetes Educators (AADE) and National Community Pharmacists Association (NCPA) Diabetes Accreditation Standards/Practical Applications (DASPA) programs to provide patient care services focused on prevention and management of diabetes and hypertension
  • Deliver care to 100 patients in Tennessee through trained and credentialed pharmacists practicing at 100 pharmacy practice sites
  • Achieve accreditation through the AADE Diabetes Education Accreditation Program for 100 pharmacy practice sites, enabling pharmacists to deliver and bill for diabetes self-management education to Medicare beneficiaries

    Eligibility

  • Participants: Pharmacists in Tennessee pharmacy practice sites
  • Participation limit: 25 pharmacists per year for 4 years
  • Site selection criteria: Targeted medically underserved areas, as defined by CDC and TDH
  • Geographic requirement: Pharmacy practice sites must be located in Tennessee

    What is Covered (2014-2015 Applicants)

  • Registration fee for one pharmacist to access AADE pre-work and attend NCPA DASPA training program
  • Financial support for pharmacists to deliver diabetes and hypertension care and management to one patient at each pharmacy practice site (payable upon successful documentation and delivery of care)
  • AADE accreditation fee (if application submitted to AADE prior to June 30, 2015)

    Requirements for 2014-2015 Participants

  • Sign up for and complete NCPA DASPA training program (November 8-9, 2014, in Nashville, Tennessee), including pre-work through AADE
  • Enroll, provide care, and document care and interventions for at least one patient within 6 months following completion of training
  • Submit application for AADE accreditation and provide confirmation of accreditation
