Award Amount: $500,000 initial funding, allocated on first-come basis to eligible practices

Eligible Applicants: Medical practices in participating states with existing electronic health record systems (EHR)

Program Description: Innovative, physician-led program to improve sharing of clinical data among physicians and hospitals through health information exchanges. HIE participants can access patient data regardless of where the patient received care.
