Geographic Scope: Wisconsin

Award Recognition: Distinguished award recognizing long-standing dedication to furthering Family Medicine specialty through excellence in care, community engagement, and support of new physicians

Evaluation Criteria:

  • Active or Life Member in good standing with WAFP

  • At least 25 years of membership in Wisconsin chapter

  • Significant impact on Family Medicine as a specialty

  • Impact on Family Medicine in the community

  • Impact on Wisconsin Academy of Family Physicians

  • Lifetime involvement in local, state, and/or national Academy and/or WAFP-Foundation activities

  • Provides compassionate and comprehensive health care service on a continuing basis

  • Demonstrates community involvement through activities that enhance quality of life

  • Exemplifies credible role model as professional in science and art of medicine

  • Fosters development and/or recruitment of new physicians in Family Medicine

  • Demonstrates balance between career and family
  • Eligibility:

  • Must be WAFP member in good standing

  • Minimum 25 years of WAFP membership required

  • Past winners cannot be re-nominated for same award but may be nominated for another WAFP award

  • Current Board of Directors members may not be considered for WAFP awards while on Board; eligible for consideration after Board term completion
  • Recognition:

  • Honored at WAFP Annual Business Meeting on Friday, March 13, 2026 at the Pfister Hotel in Milwaukee

    Nomination Process

    1. Who Can Nominate: Colleagues, peers, and others familiar with nominee's career and contributions
    2. Submission Method: Send nomination to Wisconsin Academy
    3. Nomination Deadline: July 1 (annual)
    4. Content: Nomination should comprehensively demonstrate how nominee meets all evaluation criteria with specific examples

      Required Information:

    • Nominee name and WAFP membership status
    • Verification of 25+ years WAFP membership
    • Nominator name and relationship to nominee
    • Detailed explanation of long-term contributions to Family Medicine
    • Examples of community engagement and service
    • Evidence of support and mentorship of new physicians
    • Description of impact on specialty and Academy
