FY27 Long-Term Care Provider Fund
The Long-Term Care Provider Fund will be used to improve protection of the health and safety of residents in long-term care facilities through the expansion of Long-Term Care Ombudsman Program services.
Long-Term Care Ombudsmen are advocates for residents of nursing homes and residential care communities. Ombudsmen provide information about resident rights. Under the federal Older Americans Act, every state is required to have an Ombudsman Program that addresses complaints and advocates for improvements in the long-term care system.

State Award Notices
A Notice of State Award (NOSA) shall be sent to each successful applicant using the GATA web portal. A Uniform Grant Agreement shall be sent to the Grantee once the awarded entity has signed the NOSA. The NOSA shall include estimated project start date, as well as any programmatic and financial special conditions.
Administrative and National Policy Requirements
The NOSAs shall include all necessary information regarding terms, conditions, and additional requirements of the resulting grants.
Reporting
The Grantee shall submit to the Department, program reports montly.
